Switching to “online care” involves social and technical adjustments.
Some people find it more convent and are comfortable with the experience.
Other folks, not so much.

Continuing to meet in person requires its own adjustments. Please read through and accept the form linked below if you would like to meet in person.  I’ll then contact you and we can talk through the details.

COVID-19 Consent for In-Person Psychological Services

In order to meet in person, we agree to the following details.

  • The State of Hawaii is responding to a pandemic of respiratory disease spreading from person to person caused by a novel (new) coronavirus. The disease has been named ”coronavirus disease 2019” (abbreviated “COVID-19"). This situation poses a serious public health risk. COVID-19 can cause mild to severe illness and death, particularly for people of any age with serious underlying medical problems.

    I am employing several measures to minimize the spread of COVID-19. These include, for example, social distancing, hand hygiene, clinical screening, personal protective equipment, and restricting persons with illnesses or active positive test results from entering the office. I will continue to offer the option of tele-psychology sessions.

    By signing this acknowledgement and consent, you acknowledge and accept the risks, benefits, consequences, and repercussions that could result from an incidental and unintentional exposure to COVID-19 during any in-person meetings, and that you have read the information in this document and agree to abide by its terms during our professional relationship.
